A successful candidate will have the following:
Minimum of 6 months experience working for a Home Care Agency, Nursing Home or in a Healthcare Setting.
2 Step TB within the past 12 months (or Chest X-Ray within 5 years)
Responsibilities:
Provide routine individualized care to the elderly or persons with disabilities
Monitor and report changes in health status
Provide personal care, such as bathing and dressing
Light Housekeeping
Prepare and Serve meals
Medication Reminders

What is the Community Health Choices Waiver program?
The Community Health Choices Wavier Program, often called home and community-based services (HCBS waivers), is a Medicaid program for eligible individuals who require long-term care services but prefer to stay in their homes instead of institutional settings like nursing homes. The program promotes independence, choice, and quality of life by allowing individuals to receive ongoing care while staying in the comfort of their homes.
