The Care Team Coordinator works with the Nursing Manager to ensure resident care is provided in conjunction with the mission of HFM, the needs of each individual resident and within the parameters of licensure. Supports and implements all CHFM policies, procedures and standards of practice and all clinical, behavioral and therapeutic programming as instruments to improve and/or enhance the engagement skills and quality of life for all HFM residents. In addition, the Care Team Coordinator maintains general supervision of the care staff in conjunction with the Nurse Manager. In the absence of the Administrator, Nurse Manager; the Care Team Coordinator serves as the Administrator Designee.

Essential Duties / Responsibilities:
Demonstrates an understanding of and models the mission and values of the Sisters of the Holy Family of Nazareth through behavior and attitude
Communicates with Nurse Manager on a daily basis with updates about resident care, staff and other issues
Coordinates staff scheduling in conjunction with the Nurse Manager
Serves as a professional resource to the nurse aides by coaching, advising, and mentoring
Maintains disciplinary/corrective actions of staff
Collaborates with the Nursing Manager in identifying performance improvement opportunities/evaluations
Complete weekly payroll/attendance review/weekly ADL charting audit
Maintain annual trainings per regulations for all employees
Complete new hire orientation for all employees and contractual employees
Development and adherence of break schedule for staff, dining room and chapel monitor
Makes daily rounds to ensure quality standards
Ensures communication between shifts to maintain continuity of care
Supports direct care staff with providing care and administer medications, as needed
Receives phone calls, which may include after hours to address staffing needs
Performs other duties as assigned by the Nurse Manager or Administrator
Contact with blood or other body fluids may pose a risk for exposure to blood borne pathogens and infectious diseases.
Education / Qualifications:
Med Tech Certification, CNA or Medical Assistant Preferred
Minimum of two years of experience in long term care setting
Supervisory experience preferred
Criminal background check/drug screening
Meets all health requirements imposed by law
Organized, dependable, independent thinker
Ability to lift/assist residents, physical activity such as walking, kneeling and bending for up to 75% of the time
Ability to communicate effectively both verbally and in writing
Ability to work independently and in collaboration with others
Adaptability, flexibility and leadership skills
